Solution:

The selected topic for this discussion as well as my capstone project is based on the reduction of 30-day hospital readmission rates. Specifically, the topic will focus on how hospital readmission rates can be reduced among elderly patients with congestive heart failure (CHF) by the use of a comprehensive discharge plan. On this basis, the formulated clinical question is as depicted below:

In elderly patients suffering from congestive heart failure (P), does the use of a comprehensive discharge plan (I) in comparison to the usual care (C) helps in reducing hospital readmissions (O) within a period of 30 days (T)?

As argued by Feltner et al. (2014), 30-day hospital readmissions have remained one of the most significant challenges in the care of patients with CHF.
