Requirements

Review any previous literature review you have completed related to your CGE project topic.If you have not previously completed a scholarly literature review on your topic (i.e., your topic may have changed), then you will create a new literature review with this assignment.Complete a database search on your project and compile the results, adding any new or relevant scholarly sources to your review. Focus on the most relevant articles or studies that address your problem or concern or discuss resolutions to the problem. Include any studies that do not support your projected resolution or take a different approach.In addition, search for systematic reviews about the problem or concern.Write a review of the literature or update your existing literature review using the following outline.

  1. Comprehensive review of the current evidence-based literature from nursing and related disciplines as appropriate
  2. Application of evidence-based literature to project
  3. Analysis of the literature:whether it supports your resolution; alternative solutions; how existing literature affected your proposal

Project Literature Review

Literature review is highly important in project development as it allows researchers to build on or validate the existing knowledge, avoid duplicating/ repeating research work, identify and bridge gaps in research, and resolve any conflicts in existing research (Grove, Gray, & Faan, 2019). This literature review will identify and review articles related to the PICOT question “In CHF patients (P) does the implementation of a comprehensive discharge plan (I) compared to standard care (C) decrease 30-days readmission rate from the current 20% to 10% (O) over a 12-month period (T)?” The sections included are current evidence-based literature; application of evidence-based literature; and literature analysis.

Current Evidence-Based Literature

            Following a thorough and comprehensive literature review, a total of fifteen articles related to the PICOT question were selected for review. To begin with is Jayakody et al. (2016) whose objective was to establish the methodological quality as well as the efficaciousness of telephone-based follow up intervention independently or used together with other elements in lowering 30-days readmission rates among inidviduls with diabetes, chronic respiratory disease, and cardiovascular diseases using a systematic review. The study searched articles from the Cochrane Library, MEDLINE, and EMBASE, published from May 19, 2015 onwards. The study results were inconclusive how telephone-based follow-up independently or with other components can result in reduction of readmission rates within 30 days for patients period among the study’s population of study. This warranted the need for further research on the subject.
