For a project to be successful, multiple resources must be put in place and used efficiently and effectively (Sipes, 2016). My project is about the implementation of a comprehensive discharge planning for patients with CHF in order to reduce their 30-day readmission rates with a 12-month period. The most important resource is human resources and specifically, discharge registered nurses (RNs). This is because the RNs are the frontline of the healthcare facility admission, readmission, and discharge process where they collect initial data for patient assessment and use it to drive collaborative care efforts (Keegan, Huemann, & Ringhofer, 2017).
