We are at the end of the week 8 a very challenging one but we have come thus far and I (we’re) am super happy. This week I did a rapid assessment and SBAR report to a classmate

You are in the process of admitting Ashley, a 27 year old who is 28 weeks pregnant with her first child, to the obstetric unit for complaints of a headache, dizziness, and swelling of her lower extremities when she suddenly begins seizing.

My rapid assessment would be to introduce myself to Ashley family and call for help. Ensure that oxygen and suction catheter is in the room. I would used the acronym SOAP. Subjective the pt. complaint of a headache, feeling dizzy and swelling of her extremities. Objective: V/S, 3+ pitting edema, 2+ protein in urine, blurry vision and headache on pain scale 9 out of 10. Assessment: Ensuring Ashley safety by initiating seizure precautions by making sure she is on a flat surface preferably on a blanket, if unable to get her in a bed, provide privacy, loosen any constrictive clothing and moved furniture away so she doesn’t hurt herself.
