Project Introduction
As put forward by Birmingham, Buffum, Blegen, & Lyndon (2015), it is considerably essential for healthcare organizations to have efficient and effective communication frameworks to facilitate the enhancement of care delivery and prevention of undesirable events such as medical errors, communication failures, and failure to rescue. Taking this into consideration, this paper seeks to introduce my capstone project on implementation of SBAR for communication improvement. It includes identification of problem, description of change, and benefits to nursing.
Identification of problem
I have identified the problem of communication failures due to lack of an effective communication approach whilst working as a charge nurse in the urgent care unit in Kaiser Permanente. Being a stand-alone clinic, we often experience difficulties in ensuring proper patient transfer to the nearest healthcare due to lack of reporting and communication with the receiving nurses and other medical providers. In the event a particular patient requires higher level of healthcare or hospitalization, we inform the medics or the emergency medical technicians to pick and sent our patients to Los Angeles Sunset facility where we are mandated to report to the receiving care provider.
We face difficulties in reporting off to the outside personnel as we usually don’t have information on the status of our patient and whether there has been a handoff to another nurse, and whether the handoff was appropriate. This lack of communication often results to delayed care provision and subsequently reduced health outcomes. I am certain that it is not only our healthcare organization that faces these challenges and there are many other organizations which would benefits from this project to a huge extent. The increase in medical errors, communication failures, and failures to rescue ought to be effectively addressed (Randmaa, Mårtensson, Swenne, & Engström, 2014).
