Interprofessional Teams and Patient Outcomes

The landscape of the healthcare in the United States has over the last couple of years changed substantially particularly with the introduction of the Affordable Care Act.  With the patient at the core of health care, interprofessional teams have been developed in many healthcare setting across the country. This is for the primary purpose of coordinating care as well as enhancing education for their patients (American Nurses Association, 2015).  The teams also play a fundamental role in improving overall patient health and promoting self-care.  Within my work setting, the most prevalent interprofessional teams are the palliative care teams that include doctors, nurses, pharmacists, and social workers (Moss, Seifert, & O’Sullivan, 2016).  These teams typically provide specialized nursing and medical care for people with a wide range of of life-limiting illnesses including cancer, trauma, and advanced heart disease. iCare components have been found to support interprofessional teams as patient outcomes. These components are compassion, advocacy, resilience, and evidence-based practice.  Taking this into consideration, this essay seeks to explore nursing actions can contribute to interprofessional teams in light of these components.
