The benchmark assesses the following competency:
4.2 Communicate therapeutically with patients.
The RN to BSN program at Grand Canyon University meets the requirements for clinical competencies as defined by the Commission on Collegiate Nursing Education (CCNE) and the American Association of Colleges of Nursing (AACN), using nontraditional experiences for practicing nurses. These experiences come in the form of direct and indirect care experiences in which licensed nursing students engage in learning within the context of their hospital organization, specific care discipline, and local communities.
Note: The teaching plan proposal developed in this assignment will be used to develop your Community Teaching Plan: Community Presentation due in Topic 5. You are strongly encouraged to begin working on your presentation once you have received and submitted this proposal.
Select one of the following as the focus for the teaching plan:
- Primary Prevention/Health Promotion

Community Teaching Work Plan Proposal
Planning and Topic
Directions: Develop an educational series proposal for your community using one of the following four topics:
- Bioterrorism/Disaster
- Environmental Issues
- Primary Prevention/Health Promotion
- Secondary Prevention/Screenings for a Vulnerable Population
Planning Before Teaching:
Name and Credentials of Teacher:
|
|
Estimated Time Teaching Will Last:
3 hrs |
Location of Teaching:
Community Health Center |
Supplies, Material, Equipment Needed:
Computers, projectors, and points |
Estimated Cost:
$100 |
Community and Target Aggregate:
Community members (14 years and above) |
|
Topic: Primary Prevention/Health Promotion |
Identification of Focus for Community Teaching (Topic Selection):
Promotion of healthy diet and nutrition choices through communication education to prevent and reduce metabolic syndrome.
Epidemiological Rationale for Topic (Statistics Related to Topic):
As defined by the Centers for Disease Prevention and Control (CDC) (2016), metabolic syndrome is a cluster of health conditions (high blood pressure, excess body fat around the waistline, high blood sugar, and high levels of triglyceride or cholesterol) occurring together and thus increasing one’s risk for stroke, heart disease, and diabetes type 2. Metabolic syndrome affects 22% to 30% of the U.S population (CDC, 2016). Concerning specific conditions, the rates of hypertension in U.S adults are 29.0%, affecting 30.2% and 27.7% of men and women respectively (CDC, 2017).According to the American Diabetes Association (ADA), 34.2 million Americans (i.e. 10.5%) had diabetes in 2018, with 7.3 million of the 34.2 million being undiagnosed (ADA, 2018). In the U.S, 95 million adults aged 20+ years have higher than 200 mg/dL levels of cholesterol (CDC, 2020).
Teaching Plan Criteria
Your teaching plan will be gradedbased on its effectiveness and relevance to the population selected. This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion.
Nursing Diagnosis:
High rates of metabolic syndrome and conditions.
Readiness for Learning: Identify the factors that would indicate the readiness to learn for the target aggregate. Include emotional and experiential readiness to learn.
The consumption of unhealthy foods, mainly because of the lack of knowledge about healthy diets and nutrition indicates readiness for learning.
The health belief model (HBM). This will be used to explain and also predict individual behavioral changes related to health. It will help to define and measure the factors influencing the community members health behaviors, including one’s perceived threat to disease or illness (perceived susceptibility), the belief of consequence (perceived severity), potential positive effects of action (perceived benefits), perceived barriers to action, exposure to factors prompting an action (cues to action), and confidence in one’s ability to succeed (self-efficacy) at pre and post-health promotion programs (Sulat, Prabandari, Sanusi, Hapsari, &Santoso, 2018).
Goal: Healthy People 2020 (HP2020) objective(s) utilized as the goal for the teaching. Include the appropriate objective number and rationale for using the selected HP2020 objective (use at least one objective from one of the 24 focus areas). If an HP2020 objective does not support your teaching, explain how your teaching applies to one of the two overarching HP2020 goals.
- The goal for teaching is to increase the community members’ knowledge and practice for healthy diet and nutrition behaviors for health promotion and prevention of metabolic diseases.
- D-1Reduce the annual number of new cases of diagnosed diabetes in the population (HP2020, 2020). The rationale for using this is mainly because diabetes is one of the conditions contributing to metabolic syndrome and may be prevented through the consumption of a healthy diet and nutrition.
- D-5Improve glycemic control among persons with diabetes(HP2020, 2020). The rationale for this is because diabetic people are at risk for metabolic syndrome but this risk can be reduced through the consumption of a healthy diet and nutrition.
- NWS-1Increase the number of States with nutrition standards for foods and beverages provided to preschool-aged children in child care(HP2020, 2020).
- NWS-8Increase the proportion of adults who are at a healthy weight(HP2020, 2020).The NWS-1 and NWS-8are used because the teaching focuses on improving the consumption of a healthy diet and nutrition among all populations including children and adults. Excess body fat contributes to one’s risk of metabolic syndrome.
- HRQOL/WB-1Increase the proportion of adults who self-report good or better health. The health promotion project will educate the adults and improve their self-care abilities, and thus increasing the rate of adults with good/ better health, especially concerning diet and nutrition.
- HDS-1(Developmental) Increase overall cardiovascular health in the U.S. population. Metabolic syndrome increases the population’s riskfor cardiovascular diseases and thus, the health promotion will improve people’s practice for a healthy diet and nutrition, reducing their risks for metabolic syndrome.
